We begin with these 7 photos, and from this we fill out the prescription (see form). We want you to tell us where the midline should be, the central length and shape, and how you would like the incisal embrasures. You need to include your overall goals, including the close diastema and the number of teeth you want treated.
We have now created our diagnostic model which we return to you for evaluation.
With stents in hand, you are now ready to prep the teeth and make temps. Once the patient approves the temps, you send us the impressions and the impression of the temps.
The restorations will be returned to you as shown. Please note that the final restorations are copies of the temps. You can see how they look on the day of cementation and one week later.
For this one, we received a great prescription but only 4 photos.
Please note our diagnostic work.
This dentist chose to do the upper and lower anterior teeth first. These photos show the temps and the final restorations on the day of cementation.
These images show the final result. Please note the similarities to the mock up.
The diagnostic photos and the instructions were to treat 10 upper teeth and 8 lower teeth.
These photos show the diagnostic impressions and mock up models.
